THE EFFECTIVENESS OF THE COMBINATION OF DIFFERENT METHODS OF ERGOTHERAPY IN RHEUMATOID ARTHRITIS WITH PREDOMINANT INFECTION OF THE HANDS

Анотація

Ревматоїдний артрит є поширеним захворюванням на яке тільки в 2020 році хворіли 17,6 млн. людей у всьому світі, а у 2050 році, по прогнозах, його частота може сягнути до 31,7 млн. чоловік. Найчастіше при ревматоїдному артриті уражаються міжфалангові, п’ястно-фалангові суглоби кистей. При чому уже після 2 років захворювання 67% мали принаймі одне порушення функції кистей.

З метою попередити появу нових симптомів, мінімізувати їх наслідки, покращити та підтримати функціональні можливості пацієнтів, запобігти прогресуванню деформацій, поряд з фармакологічними середниками, повинна застосовуватись ерготерапія. але потрібні додаткові дослідження щодо її ефективності , особливо комбінації терапевтичних вправ різного спрямування.

Мета дослідження – вивчити ефективність поєднання курсового застосування різних методів ерготерапії у хворих на ревматоїдний артрит з ураженням суглобів кистей.

В дослідженні прийняли участь 62 хворих на ревматоїдний артрит з переважним ураженням кистей рук. розділених на дві групи, одна з яких займалась з ерготерапевтами (31 пацієнт), а інша отримувала лише медикаментозне лікування. Для покращення функції суглобів обстежені виконували 12 сеансів тесту з 9 лунками (MART) через день та 12 разів тест Соллермана. В кінці курсу лікування їм проводилось контроль на тестування за цими методами. Також ефективність ерготерапії оцінювалась за шкалою ВАШ, показником ранкової скутості, тестом функції діяльності рук (АRAT), індексом НАQ та показником динамометрії.

Після завершення курсу комплексної ерготерапії у хворих на ревматоїдний артрит з ураженням кистей рук зменшувалась інтенсивність болю, тривалість ранкової скутості, достовірно знижувався час виконання тесту з 9 лунками та підвищення загального показника Соллермана, підвищувалась сила правої кисті, показник якості функції кисті.

Висновки. Тести з 9 лунками та Соллермана можуть бути використані не тільки як оціночні, але й як методи ерготерапії, при неодноразовому повторному виконанні. Їх тривале повторення та комбінування сприяли підвищенню амплітуди рухів в суглобах кисті, їх сили, підвищенню показників якості життя.
